New Patients

Welcome to Junction Medical. We want to be your patient centered medical home, a family practice where you can have your health care delivered in a continuous, comprehensive, and courteous manner.

We strive to affirm the following qualities of a medical home:

• "I can get my care when I need it".
• "My doctor and nurse know me".
• "My office provides for most of my needs".
• "They coordinate care when I need additional services".

Your initial visit will last 30 to 60 minutes. In order to make use of our visit time, we request you arrive 15 minutes early so basic registration information may be completed.

• Complete the Patient Registration form (required).
• Privacy Policy (HIPAA) with signed acknowledgment (required).
• Email policy with signed consent (optional).
• Personal family health history (optional but helpful).

For your convenience, our forms may be down loaded and printed so they may be faxed or presented at your visit.

Please bring your insurance card to every visit.

TRANSFER OF RECORDS
It is always helpful to have a copy of your previous medical records for review. A Transfer of Records form is available to download and print directly from our website. You may take this form to your previous physician's office and request a copy of the records be sent directly to us. If the charge for this service appears to be prohibitive, please check with our office and we will outline only those records that are considered to be essential. Examples include: recent diagnostic testing results, recent lab work, and a summary of all your active and past medical issues.

The Transfer of Records form may also be signed at the time of your visit at our office.
