May Practice News
This year May not only brings flowers but also a new public health concern about H1N1 influenza (also known as Swine Flu). The unique genetic strain of this virus has raised concern about a new worldwide flu pandemic similar to 1918 (50 million fatalities worldwide). These fears seem to have subsided but why the H1N1 seems so much more deadly when contacted in Mexico has yet to be determined.
In the meantime, practice frequent hand washing and keep abreast of additional recommendations of our public health departments and the CDC. As of today, there are no confirmed cases in Peoria and no need to panic.
In last month's newsletter, I discussed the importance of tracking, measuring and reporting office based quality indicators. Patient Centered Medical Homes must demonstrate their "value" if this new model of practice is to grown and succeed.
So here are our first two sets of measures - more will be forthcoming as we add new patients and increase our data points.
A. Patient Services - Notification of test results.
Measure: Percentage of patients who we notified of hospital based results (XR, heart studies, CT
scans, etc.) within 24 hours of test completion.
Results: For period 1/1/09 - 4/30/09 16/24 = 66% (Target 75%).
Comments: You would be surprised (well, probably not) by the number of steps and people involved in the process of getting a test result to you in a timely manner. Marilyn and I are communicating with hospital departments in an effort to overcome obstacles and "make the system work for you".
B. Clinical Care - Blood pressure control.
Measure: Percentage of patients with diagnosis of hypertension whose most recent BP is less than 140/90.
Results: For period 1/1/09 - 4/30/09 25/61 = 41% (Target 75%)
Comments: We have a way to go before our target is achieved. I am asking patients to focus on diet (especially sodium) and exercise in addition to medication management. I believe we will see improved levels of control as our new patients come in for their follow up visits.
