Please familiarize yourself with these specific covered and non-covered services of your insurance plan.

 

CHARGES AND BILLING

Our office will submit your bill to your insurance carrier IF we have the correct information available to us.  Please bring your insurance card to every visit.

Please pay your co-insurance or co-pay at the time of visit (amount due is usually found on your insurance card).  In order to improve the efficiency of our practice, please make co-payment at the time you sign in.

If you do not have insurance coverage and pay at time of service, your bill will be discounted 25%.
