MAKING AN APPOINTMENT
You can request an appointment in one of two ways:

1.    Call our office at 740-4232.  If we are not available to answer your call promptly, you may leave a message on our voicemail.  Please include your name a call back phone number.
2.    You can also request an appointment utilizing e-mail.

URGENCIES AND EMERGENCIES
If you need to reach the physician on an urgent basis when our office is closed, simply call 740-4232 and dial ‘0’ when prompted.  This will direct your phone call directly to the physician’s cell phone.

In case of an emergency, please dial 911.  Please make sure you provide the paramedic response team and the emergency department with our name.  We will then be able to forward valuable information from your electronic health record.

OPEN ACCESS SCHEDULING
Our office is committed to providing “open access scheduling.”  We will offer you a same day appointment regardless of the reason for your request.  You do not have to convince us why you need to be seen today.  We trust your judgment and value your time.  In order to accomplish “open access,” we will keep a certain number of our appointment slots open so that we can accommodate requests for same day visits.  If you request a same day visit, please contact us as early in the day as possible. 

If you leave us a message on our phone line, we will make every effort to respond to morning calls before 1:00 p.m. and afternoon calls before 6:00 p.m.  You may also use e-mail to request an appointment.  We are committed to responding to all e-mail messages within a 24 hour period.

HOUSE CALLS
We do provide visits for home bound patients. Please call our office and this will be arranged on an individual basis. At this time, home visits are limited to zip codes 61614 and 61615.
